Coarctation of the aorta (CoA) is a common congenital anomaly that is usually treated in infancy or childhood. Adult patients with coarctation have a high incidence of associated cardiac disorders, including valve diseases, atrial fibrillation and ischemic heart disease. Most patients with uncorrected CoA die before reaching the age of 50 from complications such as myocardial infarction, intracranial hemorrhage, congestive heart failure (HF), infective endocarditis or aortic dissection.

The case of patient with advanced congestive heart failure, NYHA III, of ischaemic and valvular aetiology and concomitant diseases is presented. Introduction of 6-month, controlled physical training resulted in improvement of health status, exercise performance, ventilation and left ventricular function. Quality of life got significantly better. This aspect of treatment should be considered in majority of patients with heart failure.

We present a case of 80-year-old man with chronic atrial fibrillation and heart failure with mildly depressed left ventricular ejection fraction who deteriorated after implantation of pacemaker to right ventricular apex. The patient improved when pacemaker was upgraded to resynchronisation therapy (CRT). The question is raised if CRT should have been implanted primarily.

Peripartum cardiomyopathy (PPM) is an acquired disease of increasing incidence. Its diagnosis and treatment is often problematic. We report a case of 31-year-old woman who was diagnosed with PPM three weeks after delivery and died nine months later despite proper therapy.

The kidney controls the volume of blood that is required for proper perfusion of organs.
